Release and Waiver
Scott Kim Tennis Academy. – TENNIS INSTRUCTION AGREEMENT * This form must be completed before commencing ANY tennis instruction or related activities.
I hereby release Scott Kim Tennis Academy, its instructors, agents, servants, assignees, employees, or volunteers from any liability or responsibility for any death, injuries said minor in my custody might sustain while participating in any activity connected in any way with the aforementioned program. Additionally, I agree that neither I nor my spouse, assignees, heirs, guardians, and/or legal representatives will make any claim against, sue or attach the property of Scott Kim Tennis Academy and/or any owner or agent thereof, for any loss or damage resulting from my participation in the Activity.
I understand that my child may be transported by volunteer drivers.
If I am injured, I agree to assume any financial obligation, either through my personal health insurance, or through some other means, for any medical costs which I incur. Scott Kim Tennis Academy assumes no responsibility for any medical expenses, injury, or damage suffered by Participant in connection with the use of any Scott Kim Tennis Academy facilities or services in connection with the Activity (regardless of where such services take place)
Consent to Treatment of a Minor: In the event of sudden illness, accident or injury which may occur while said minor is engaged in any activity supervised by Scott Kim Tennis Academy staff and its representatives, agents, or assignees, when neither the parent nor the guardian can be contacted. I hereby give my consent to California Family Code Section No. 6910 for emergency medical treatment. In the event of urgency according to the circumstances medical treatment may be given by any doctor licensed under the laws of the State of California.
PHOTOGRAPHY CONSENT, RELEASE AND WAIVER OF LIABILITY
I hereby give my consent to the Scott Kim Tennis Academy to photograph me and use the photograph(s) for informational, educational, promotional, or publicity purposes concerning the academy and its services.
I understand that the photograph(s) may be used on the Scott Kim Tennis Academy’s Website, or in official county publications or displays, public newspapers, magazines, reports, or other public documents; or electronic or digital recordings. I also understand that the photograph(s) may be used without any further consent or authorization from me; Scott Kim Tennis Academy may modify the photograph(s) in the process of editing, and I will not be entitled to any compensation for use of the photograph(s).
I also agree to release Scott Kim Tennis Academy, its officers, employees, or agents, from any and all liability arising out of or connected to the use of the photograph(s) as stated above.
I have read and understand the foregoing consent, release, and waiver of liability, and voluntarily accept and agree to its terms.
